SIWES Forms Explained: ITF Form 8 and SCAF

Students search for "SIWES form C" and "SIWES assessment form" and end up more confused. Different people use different names, and some forms look alike. Here are the main forms in plain terms, so you know what to ask for and who fills what.

Sources describe these forms a little differently, and schools can add their own, so confirm the details with your SIWES coordinator or the ITF guideline for your session.

The short version

FormWhat it isWho fills it
LogbookDaily and weekly record of your trainingYou, signed by supervisors
ITF Form 8End-of-programme report sheet (assessment)Student (Part A), employer (Part B), institution (Part C)
SCAFForm that records the start of your attachmentReported as filled with the employer, then sent to ITF

The logbook is not an ITF form, but you fill it alongside Form 8. See how to fill your SIWES logbook.

ITF Form 8: End-of-Programme Report Sheet

Form 8 is the main document used to assess your SIWES. It is generally described as having three parts.

Part A: the student

You fill in your personal details and training details, such as your name, matric number, institution, course, the company and the period of your training. Write clearly and check every detail against your documents, because mistakes can delay processing.

Part B: the employer

Your industry-based supervisor or the company assesses your performance and gives feedback. This normally covers things like punctuality, attitude, skills shown and overall performance. Ask early who at the company will complete it, and remind them before your last week.

Part C: the institution

Your school's side. This is where the institution records its evaluation of the training and of your involvement, and where visits and grades are recorded. Sources differ slightly on the exact focus of Part C, but they agree on the three-part structure. When students talk about "SIWES Form C", they are usually referring to this part of Form 8 or to the school's own assessment form.

For a step-by-step guide to filling it, read how to fill ITF Form 8.

Why Form 8 matters

Reports describe Form 8 as required for SIWES approval. They also say that when it is not submitted, supervisors' allowances are not paid. Students who do not submit it on time may also have delays with their SIWES result. Ask your SIWES unit for the exact deadline and where to hand it in.

SCAF: Student Commencement Attachment Form

The SCAF records the start of your placement so ITF can monitor it. Sources describe it as a form sent to the ITF Area Office nearest your place of training, and some say it applies to 6-month programmes. Who fills it in and when is described differently in different places, so ask your SIWES unit at orientation.

It is not an assessment and it is not your logbook.

Logbook, report and forms: how they fit together

  1. Before you start: get your placement letter and acceptance letter. See how to get a SIWES placement.
  2. At the start: complete the commencement form (SCAF) if your school or ITF requires it.
  3. During training: fill your logbook daily and get it signed weekly.
  4. At the end: your employer completes their part of Form 8, you complete yours, and the school completes its part.
  5. After training: submit your technical report. See how to write a SIWES report.

Common mistakes

  • Waiting until the last day to find your employer. Busy supervisors may take days to complete their part.
  • Wrong or missing details. Check your name, matric number and dates.
  • Unsigned or unstamped forms. Ask what stamp or signature your school requires.
  • Losing the form. Keep copies of everything.
  • Mixing up the logbook and Form 8.

Tools that save time

None of these replace the official forms. You still collect the real signatures and stamps.
